Job summary

Qualifications

• Bachelor's degree in a relevant field (social work, psychology, or human services) is required.
• Knowledge of DHS 36 and related program codes is preferred.
• Experience in CCS or community-based services is desirable.
• Ability to document thoroughly in electronic records and reports.

Responsibilities

• Provide comprehensive, person-centered case management and service facilitation.
• Coordinate and monitor treatment plans with a multidisciplinary team.
• Advocate for community resources to support natural living environments.
• Document consumer contacts and time in the designated system; ensure accuracy for billing.
• Attend trainings and meetings as required by the supervisor.
