Job summary

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• One year of experience working with individuals experiencing mental illness and/or substance use.
• Valid driver’s license, automobile, and insurance per agency requirements.
• Passion for mental health advocacy and belief in recovery and harm reduction models.
• Must meet all employee requirements including background checks and driving record.

Responsibilities

• Build trusting relationships with individuals and meet them where they are.
• Coordinate supports using a trauma-informed approach.
• Develop and update individualized recovery plans with the team.
• Communicate with providers to ensure coordinated care.
• Document interactions accurately and in a timely manner.
• Monitor progress toward clients' goals and provide coaching.
• Assist with transportation to program-related appointments.
• Respect confidentiality and adhere to policies.
