Job summary

The Salvation Army North and Central Illinois Division is seeking a Case Manager for the Safe Harbor Shelter in Bloomington, IL. You will provide comprehensive case management, assess client needs, develop and monitor case plans, act as a liaison to community resources, and arrange for client transportation.

You will document client information in HMIS, submit required reports to DHS, HUD, and The Salvation Army, and participate in monthly meetings.

Qualifications

• Bachelor of Science degree in social work, sociology, criminal justice or another related field; or five years’ experience in social work or case management.
• Minimum of 3 years experience working with homelessness or vulnerable populations.
• Strong documentation, timely reporting, and compliance with program requirements.

Responsibilities

• Provide case management services including intake review and needs assessment.
• Develop and monitor client case plans and coordinate community referrals.
• Arrange transportation for clients and advocate for their needs.
• Document progress in HMIS and ensure data accuracy.
• Submit required reports to DHS, HUD, and The Salvation Army.
• Attend CIOC meetings and participate in staff collaborations.
• Maintain client records with confidentiality and professionalism.
