Job summary

The Salvation Army North and Central Illinois Division is seeking a dedicated case manager to provide comprehensive services to clients experiencing homelessness and other vulnerable populations. You will review intake information, assess needs, develop and monitor case plans, coordinate with community resources, and arrange transportation as needed.

Qualifications

• Bachelor of Science in social work, sociology, criminal justice or related field; five years in social work acceptable in lieu of degree.
• 3+ years experience with homelessness or vulnerable populations; strong documentation and compliance focus.

Responsibilities

• Review intake for accuracy, assess needs, develop case plans and ensure compliance.
• Serve as liaison to community resources and referrals; arrange client transportation.
• Maintain client records ethically and confidentially; document progress in HMIS Service Point.
• Attend Continuum of Care meetings and assist with discharge planning.
• Ensure accessibility to a case manager via appointments or walk-ins.
• Record and report statistics to comply with DHS, HUD, and Salvation Army requirements.
