Job summary

The Salvation Army Midland Division Alton Shelter in Alton, IL is seeking a Shelter Case Manager to provide case management services to shelter clients and ensure compliance with grant requirements. The role does not supervise staff and is full-time (40 hours/week).

Responsibilities include intake assessments, goal setting, coordinating housing resources, maintaining SIMS data, referring clients to services, and reporting outcomes for funders.

Qualifications

• Bachelor’s degree in social work or related field with at least 1 year of human services experience.
• Strong interpersonal and communication skills.
• Ability to type case notes and use Word/Excel; HMIS training to be completed within 3 months.

Responsibilities

• Conduct initial intake assessments to determine eligibility and client needs.
• Provide case management using Pathway of Hope and grant requirements.
• Develop and monitor action plans and follow-up meetings with clients.
• Identify housing options and coordinate referrals to services.
• Enter client data into SIMS and maintain confidential records.
• Distribute resources and coordinate with community agencies.
