Job summary

The Salvation Army North and Central Illinois Division is seeking a part-time Case Manager to provide outreach, intake, needs assessment, direct assistance, and discharge planning. You will develop and monitor client case plans, coordinate referrals, and ensure compliance with DHS, HUD, United Way, FEMA, and funder standards.

You will document progress in HMIS/SIMS by WellSky or other required databases, attend monthly COC meetings, and advocate for clients while maintaining ethical records and

Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.
• Experience in Social Services and case management is preferred.
• Proficiency with computer software including MS Office and case management databases.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals.
• Provide case management services including assessment, direct assistance, case plan development and follow-up.
• Maintain client records and ensure confidentiality and compliance with policies.
