Job summary

PATH in San Diego, CA is seeking a Case Manager to support clients in breaking the cycle of homelessness with individualized service planning and access to housing and services. This full-time, non-exempt role requires driving and operates in a multi-site environment.

Qualifications

• Two years of experience in healthcare, case management, homeless services or related field.
• Experience working with vulnerable populations preferred.
• Ability to work with diverse teams and coordinate services.

Responsibilities

• Conduct intake and individualized needs assessment to develop ISP addressing housing goals and income.
• Provides referrals and linkages to resources to achieve ISP goals.
• Completes program-specific assessments (e.g. VI-SPDAT).
• Evaluates progress with 90-day reviews after intake.
