Responsibilities

• Interview applicants to determine eligibility for program enrollment.
• Develop intake and case plans for clients.
• Enter information into HMIS and maintain case records.
• Plan and facilitate life management classes.
• Assist clients in locating affordable housing.
• Complete discharge plans and follow-up.
