Qualifications

• Education:Bachelor's degree in Social Work or related human service field; relevant experience in lieu of education may be considered.
• Experience: One year of case management experience.
• Certifications/Licenses: First Aid and CPR; if not certified, obtain within 90 days of hire.

Responsibilities

• Conduct client interviews to determine level of need.
• Assess client’s psychological, emotional, medical, financial, spiritual, and substance abuse status.
• Complete paperwork and submit to program manager for weekly review.
• Complete orientation paperwork within 48 hours of admission.
• Attend weekly house meetings and facilitate when needed.
• Monitor client activities to maintain a substance-free environment.
• Document all sessions and incidents; report to program manager.
• Maintain client case files and document progress daily.
• Provide transportation to appointments as needed.
• Provide reports and statistical information as directed.
• Stay informed on resources to provide referrals.
