Job summary

The Salvation Army A Georgia Corp in Texas is seeking a qualified case manager to interview clients, assess eligibility, and deliver long-term, structured case management for an assigned caseload within an established life management program.

You will develop comprehensive program goals, mentor and counsel clients, advocate for services, and maintain accurate records while ensuring compliance with funding requirements.

Qualifications

• Requires a Bachelor's degree in Social Work or related field and relevant experience.
• Minimum three years of progressively responsible case-management experience.
• Ability to develop long-term action plans for recovery and coordinate resources.

Responsibilities

• Interview clients to determine eligibility based on program guidelines.
• Develop long-term program goals and monitor progress with counseling.
• Advocate for clients to access needed services.
• Maintain accurate case records and HMIS entries.
• Assist clients with completing applications for assistance.
• Ensure compliance with funding requirements.
