Job summary

The Salvation Army Southern California is seeking a Case Manager to provide ongoing assessment, planning, implementation and evaluation of client needs, with the goal of securing permanent housing and promoting cost-effective care. The role reports to the Program Manager and emphasizes intensive case management, data entry in HMIS, and collaboration with community partners.

Qualifications

Bachelor of Arts in Social Work preferred or Associates Degree or related field preferred.
Minimum of one-year experience working with low-income and/or homeless individuals and families.
Must possess a Valid Class C California Driver’s License.
Ability to read, write, speak and understand English.
Bilingual English/Spanish preferred.

Responsibilities

Provide strength-based case management and service coordination to help clients obtain and maintain permanent housing.
Update and maintain client information per HMIS; ensure data accuracy and reporting.
Develop and monitor individual service plans and conduct re-evaluations as needed.
Assist clients in accessing financial entitlements and health care services.
Adhere to program policies using evidence-based practices and housing-first approaches.
Coordinate with partner agencies to maximize participant outcomes; attend weekly case management meetings.
