Job summary

Qualifications

• Bachelor's degree in Social Work or related field required or equivalent
• Three years of progressively responsible experience in direct case management services
• Experience developing individualized, long-term recovery plans and coordinating resources

Responsibilities

• Provide comprehensive case management for an assigned caseload in a life management program
• Assess client needs and develop long-term program goals and progress evaluations
• Advocate for clients to access services and resources
• Coordinate with external counseling resources as needed
• Assist clients with obtaining income, benefits and other supports
• Maintain up-to-date records and HMIS data; document progress and outcomes
• Conduct regular client meetings to review progress and motivate positive change
• Plan and facilitate life management and addiction-related classes to build coping skills
• Ensure program compliance with funding requirements and reporting standards
• Refer clients to Corps programs or other agencies as appropriate
• Complete discharge plans and maintain confidentiality of records
