Job summary

Phoenix House of New York seeks a dedicated Case Manager to support shelter residents. You will conduct intake, assess needs, identify benefits, and guide clients toward employment and housing resources.

Bililngual preferred; familiarity with CARES is a plus. We offer comprehensive benefits including employer-paid medical insurance for eligible employees and dependents.

Qualifications

• Bachelor's degree in human services and at least 1 year of experience in a human service field.
• An associate degree with 2–3 years of relevant experience is acceptable.
• Bilingual abilities are preferred and CARES familiarity is a plus.

Responsibilities

• Conduct initial intake and psychosocial assessments for new shelter clients.
• Update evaluations to guide referrals and interventions.
• Identify and connect clients to benefits, services, and housing resources.
• Obtain identification and public benefits for clients.
• Hold regular meetings to finalize living plans and track needs.
• Document all client interactions and outcomes in records.
• Manage employment and housing placements for residents.
• Maintain confidential case records and complete trainings.
