Qualifications

  • Bachelor's degree in a human services field with 1+ year of relevant experience.
  • Associate degree with 2–3 years of relevant experience in human services.
  • Bi-lingual preferred.
  • Familiarity with CARES is preferred.
  • Experience with NYC Shelter System and DHS is preferred.
  • Strong communication, empathy and cultural competency are essential.

Responsibilities

  • Conduct initial intake, including social service and psychosocial assessments and referrals.
  • Update psychosocial assessments to guide referrals and interventions.
  • Identify benefits and entitlements and connect clients to services.
  • Assist clients in obtaining IDs, public benefits, and income sources.
  • Lead weekly/bi-weekly meetings to finalize living plans and coordinate services.
  • Document all services and follow-up in client records.
  • Track employment and housing placements for shelter residents.
  • Maintain confidential case records and complete trainings timely.

You will have responsibilities including but not limited to:
  • Conduct initial intake, including but not limited to social service assessments, psychosocial assessments, independent living plans, and referrals for incoming clients who are referred from the shelter system.
  • Conduct updates to psychosocial assessments and other evaluations as a basis for developing appropriate referrals and other interventions.
  • Identify benefits and entitlements and connect clients to mental health, medical treatment, and substance use treatment facilities where applicable.
  • Obtain identification, public benefits, and other applicable income sources for clients.
  • Conduct weekly and bi-weekly meetings with clients to finalize their Individual Living Plans, assess their ongoing needs, promote harm reduction strategies, and facilitate referrals for necessary services.
  • Document all efforts in providing services, interactions, and follow-up regarding resident involvement.
  • Manage and track employment and housing placements of shelter residents.
  • Maintain current and confidential client case records for the assigned caseload.
  • Complete all required trainings in a timely manner.
Job Details:

Homeless Shelter Jamaica- Queens, NY

Full Time-non-exempt

Salary range $24.00 - $26.00 hourly

QualificationsQualification:
  • Degree Requirement: Bachelors relevant to human services and at least one (1) year of proven experience in Human Service-related field or.
  • An associate degree with at least two (2) – three (3) years of proven experience in a Human Service-related field.
  • Bi-lingual preferred.
  • Familiarity with CARES, preferred.
  • Familiarity with NYC Shelter System with any population; Families with children, single adults, mental health, substance use, HIV/AIDS, or any other related field.
  • The ideal candidate will demonstrate sensitivity, tact, and resourcefulness.
  • A strong desire to assist clients in a nonjudgmental, empathetic, and compassionate manner is essential.
  • The capacity to effectively address cultural distinctions and a dedication to the importance of cultural competency are essential.
  • The ability to establish and maintain appropriate professional boundaries.
  • Demonstrates exceptional organizational, time management, and written communication abilities.
  • Experience working with DHS and other service providers, Preferred.
  • Experience providing case management services to families with children/Single Adults referred from DHS.
Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.

Must be able to lift to 25 pounds at times

Must be able to access and navigate each department at the organization’s facilities.
