Heading Home is seeking a Case Manager Supervisor to lead a team of case managers dedicated to serving clients experiencing homelessness. The role emphasizes Housing First, trauma-informed care, and motivational interviewing while ensuring service delivery aligns with contractual and regulatory standards.

The supervisor will oversee performance, provide coaching, and collaborate with leadership to strengthen client care and program operations.

Mansioni

• Supervise a team of case managers and ensure best practices.
• Screen referrals and provide one-time assistance as needed.
• Provide transportation support to clients as required.
• Coordinate with Program Director to align services with contracts and standards.
• Promote trauma-informed care across the team.
• Monitor assessments, service plans, and case notes for quality and compliance.
• Conduct supervision meetings, evaluations, and coaching sessions.
• Plan and evaluate programs with leadership to improve client care.
• Maintain staffing coverage and approve time off.
• Onboard and train new case management staff.
