Job summary

Key Works is seeking a Case Manager Supervisor to provide leadership and operational coordination for Case Management and Medical Case Management teams. The role ensures effective delivery of services, supports staff development, and collaborates with clinical leadership to optimize patient care from treatment through community transition.

The position involves supervising staff, monitoring performance, and ensuring documentation compliance while coordinating with multiple departments to close

Qualifications

• Must have AA Degree in SUD studies or Bachelor's Degree (preferred).
• Must have experience working with patients with SUD and co-occurring disabilities.
• Computer and EHR literate.

Responsibilities

• Provide direct supervision to Case Managers and Medical Case Managers.
• Monitor staff productivity, quality of services, and documentation.
• Coordinate with Clinical, Health Team, Admissions and other departments to ensure effective communication.
• Assist with complex discharge planning and transitions to community services.
