Job summary

Community Housing Innovations (CHI) is seeking a Case Management Supervisor to lead a team of case managers and oversee the development and execution of service and care plans for residents. You will coordinate resources, develop partnerships with community-based services, and provide direct supervision to your team.

Responsibilities include reviewing plans, ensuring eligibility for public benefits, and maintaining a strong network of external providers.

Qualifications

• Bachelor’s degree in Social Work or Psychology is required.
• Five+ years of experience plus two years of supervisory experience in social services.
• Seven+ years of case management experience with three+ years supervisory experience in past ten years may substitute for educational requirement.
• Proficiency in English required; multilingual and Spanish prioritized.
• Computer skills including MS Word, Excel, Outlook; CARES/HOME/CAPS/AWARDS prioritized.
• Experience working with homeless individuals and people with health/mental health issues.
• Ability to engage residents and build partnerships with community organizations.

Responsibilities

• Coordinate service and care plans to support residents and promote independence.
• Review and customize care plans to residents’ biopsychosocial needs.
• Develop partnerships with external providers for immigration, health, education, and employment services.
• Ensure residents access public benefits and city programs per DHS requirements.
• Supervise Case Managers and provide coaching, feedback, and evaluations.
