Job summary

Key Works is seeking a Case Manager Supervisor to lead Case Management and Medical Case Management teams, coordinating resources, discharge planning, and service delivery for patients. You will supervise staff, monitor quality, and collaborate with clinical and operational leadership to close care gaps.

Qualifications

• Must apply for RAAC credentials within 30 days of hire.
• Must apply for MAR credentials within 30 days of hire.
• CPR/First Aid certification, within 30 days of hiring.
• Valid WA driver's license and clean driving record.
• Ability to pass background checks and drug screening.
• Strong organizational, interpersonal, conflict resolution, and problem-solving skills.
• Proficiency with electronic health records, clinical documentation, and performance monitoring.

Responsibilities

• Provide direct supervision and support to Case Managers and Medical Case Managers.
• Establish performance expectations and distribute workloads and patient assignments.
• Conduct regular supervision and team meetings to review performance and promote consistent practices.
• Monitor staff productivity and quality of services; provide coaching and corrective feedback.
• Conduct performance evaluations and support staff development.
• Address staff performance concerns and coordinate with leadership and HR.
• Monitor case management documentation for accuracy and timeliness; ensure compliance.
• Identify operational inefficiencies and implement process improvements to strengthen service delivery.
• Coordinate with Clinical, Health Team, Admissions, and other departments to ensure effective communication and service coordination.
• Participate in hiring, onboarding, training, and evaluation of Case Management staff.
• Ensure departmental compliance with policies, confidentiality, and regulatory standards.
