Responsibilities

• Evaluate members’ needs/eligibility and recommend care plan actions.
• Identify high risk factors and escalate to appropriate care management.
• Coordinate and monitor care plan activities and progress.
• Support quality of care through collaboration with medical directors and case managers.
• Identify and escalate quality issues through established channels.
• Negotiate options and services to meet member benefits.
• Engage members to promote healthy lifestyle changes.
• Mentor and coach team members; participate in meetings.
