Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field
• Willing to travel 50-75% in the Metro East, Illinois area; reliable transportation required
• 2+ years of electronic documentation experience and Microsoft Office usage

Responsabilités

• Evaluate members’ needs/eligibility and propose care plan actions
• Identify high risk factors and coordinate appropriate referrals
• Coordinate and monitor care plan activities and progress
• Collaborate with care teams to improve quality of care and outcomes
• Escalate quality of care issues through proper channels
• Engage members to promote healthy lifestyle choices and adherence
