• Coordinate and monitor assigned care plan activities.
• Identify high-risk factors and refer to clinical care management or crisis intervention as needed.
• Collaborate with care teams to overcome barriers and optimize care outcomes.
• Document and escalate quality of care issues following regulatory requirements.
