Job summary

CVS Health is seeking a Case Management Coordinator for a full-time telework role with Illinois-based travel requirements. This position supports members by coordinating care, scheduling appointments, and guiding access to benefits.

The role requires a Bachelor’s or non-licensed master-level clinician credential in Behavioral Health or Human Services and at least 2 years of related experience, including electronic documentation and Microsoft Office proficiency.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Must travel 50-75% within Mason, Cass, Scott and Brown counties, Illinois. Reliable transportation.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and propose case resolution strategies.
• Identify high risk factors and coordinate referrals to clinical case management or crisis intervention as needed.
• Coordinate and monitor care plan activities and progress.
• Consult with medical directors and others to improve quality of care and overcome barriers.
• Escalate quality-of-care issues through established channels.
• Engage with members to promote informed healthcare decisions and lifestyle changes.
