Qualifications

• Minimum 2+ years in Behavioral Health or related field.
• Proficient with electronic documentation systems and Microsoft Office.
• Willingness to travel 50-75% in Will, Kankakee and Grundy Counties, IL.

Responsibilities

• Evaluate members’ needs and eligibility to recommend care plans.
• Identify high-risk factors and refer to clinical case management when needed.
• Coordinate and monitor care plan activities and progress.
• Collaborate with Medical Directors and other programs to improve care outcomes.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Coach members to engage in healthy lifestyle choices.
• Participate in team meetings and mentor peers.
• Assist members in collaborating with providers in decision-making.
• Document care activities per regulatory and company policy.
