Job summary

Adoration Health seeks a Care Transition Coordinator to facilitate smooth transitions from facilities to home health or hospice. You will coordinate patient eligibility, care plans, and ancillary services, while serving as a liaison with referral sources and providers to ensure timely communication and documentation.

Qualifications

• Minimum of one year of home health or hospital-based case management experience.
• Preferred: 1–3 years of medical marketing or healthcare business development.
• Current and active licensure in the state of practice as RN/LPN/SW/PT is required.
• Respiratory Therapist certification and/or related clinical program knowledge is preferred.
• Must possess a valid driver’s license, reliable transportation, and current auto insurance.
• Demonstrated understanding of home health eligibility criteria and Medicare coverage guidelines.

Responsibilities

• Achieve monthly production goals and MC admission targets for assigned locations.
• Implement sales and market strategies to increase market share.
• Evaluate patient eligibility for home care per Right of Choice guidelines.
• Conduct face-to-face transitions to educate patients and identify responsible physician.
• Coordinate transfer orders and ancillary services (DME, infusion) after patient acceptance.
• Educate patients on post-discharge care and medication regimens.
• Act as liaison between the agency and healthcare providers for referrals and transfers.
• Notify discharge planning of active patients transferred from home health to a facility.
