Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice. You will evaluate eligibility, coordinate care plans, and arrange services including DME and infusion.

This role acts as a liaison among the agency, referrals, and healthcare providers, ensuring timely documentation and patient education. The position supports market growth, maintains financial stewardship, and emphasizes high-quality, personalized transitions

Qualifications

• Minimum 1 year in home health or hospital-based case management.
• RN/LPN/SW/PT licensure required; RT is preferred.
• Must possess valid driver’s license and reliable transportation.

Responsibilities

• Achieve monthly personal production goals and MC admission targets for assigned locations.
• Coordinate care plans and arrange services including DME and infusion.
• Educate patients and coordinate with discharge planners and physicians; ensure compliance with agency protocols.
• Present identified patient needs to leadership for branch approval and ensure proper documentation in patient records.
• Coordinate transfers and resumption of care with referrals and providers; ensure timely communication.
