Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate transitions from facilities to home health or hospice care. You will evaluate eligibility, coordinate care plans, and arrange DME/infusion services in line with agency protocols.

Qualifications

Minimum of 1 year of experience in home health or hospital-based case management.
Current and active licensure as RN, LPN, SW, or PT in the practicing state.
Knowledge of Medicare/insurance coverage guidelines and patient eligibility criteria.

Responsibilities

Achieve monthly production goals and MC admission targets for assigned locations.
Coordinate care plans and arrange ancillary services per agency protocols.
Serve as liaison between the agency, referral sources, and healthcare providers.
Educate patients on post-discharge plans and medication regimens.
Document encounters and manage transfer orders and resumption of care.
Maintain patient confidentiality and compliance with policies.
