Adoration Health, an affiliate of BrightSpring Health Services, seeks a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice, coordinate eligibility, care plans, and arrange services in line with agency protocols.

The role serves as a liaison between the agency, referral sources, and healthcare providers, ensuring timely communication, documentation, and patient education to support market growth and compliance.

Competenze

• Minimum of one (1) year of experience in home health or hospital-based case management.
• One (1) to three (3) years of experience in medical marketing or healthcare business development is preferred.
• Current and active licensure in the state of practice as an RN, LPN, SW, or PT is required.
• Respiratory Therapist certification is preferred.
• Must have a valid driver's license, reliable transportation, and current auto insurance.
• Knowledge of home health eligibility criteria and Medicare/insurance coverage guidelines is required.

• Achieve monthly personal production goals and MC admission targets for assigned locations.
• Implement strategies to increase market share within facilities.
• Evaluate patients and orders for home care eligibility per guidelines.
• Conduct face-to-face patient transitions and educate on plan of care.
• Coordinate transfer orders and ancillary services; educate on orders and services.
• Ensure all referral source needs are documented and addressed.
• Collaborate with leadership to promote growth and align with referral sources and patients.
• Perform sales administration duties and maintain timely communication.
• Educate patients on post-discharge physician appointments and medication regimens.
• Act as liaison between agency and healthcare providers for referrals.
• Notify discharge planning of active patients; coordinate resumption of care when applicable.
