Qualifications

• Minimum of one year of experience in home health or hospital-based case management.
• Preferred: 1–3 years in medical marketing or healthcare business development.
• Current and active licensure in the state of practice as RN, LPN, SW, or PT.
• Must have a valid drivers license, reliable transportation, and current auto insurance.
• Understanding of home health eligibility criteria and Medicare guidelines.

Responsibilities

• Achieve monthly personal production goals and Medicare-certified (MC) admission targets for assigned locations.
• Coordinate transfer orders and ancillary services (e.g., DME, infusion).
• Educate patients on post-discharge physician appointments, prescriptions, and medication regimens.
• Act as liaison between the agency and healthcare providers for new referrals and patients transferred from home health to hospitals.
• Collaborate with leadership to promote growth by aligning team efforts with referral sources and patient needs.
