Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice. You will evaluate eligibility, coordinate care plans, and arrange DME/infusion services, while educating patients and families about plans of care.

Qualifications

• Minimum of 1 year in home health or hospital-based case management.
• Preferred 1–3 years in medical marketing or healthcare business development.
• Current active licensure in RN/LPN/SW/PT is required; RT cert is preferred.

Responsibilities

• Achieve monthly production goals and MC admission targets for assigned locations.
• Coordinate care plans and arrange ancillary services per guidelines.
• Serve as liaison between agency, referral sources, and providers.
• Educate patients and families on post-discharge plans and medications.
• Document encounters in the agency system and ensure compliance.
