Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice care. You will evaluate eligibility, coordinate plans, and arrange services including DME and infusion, while liaising with referral sources and providers.

You will drive growth through outreach, maintain compliance and patient education, and handle related sales administration tasks as part of a broader care coordination team.

Qualifications

• Minimum of 1 year of experience in home health or hospital-based case management.
• Current active licensure in state of practice as RN/LPN/SW/PT.
• Valid driver’s license with reliable transportation.

Responsibilities

• Meet monthly production goals and MC admission targets for assigned locations.
• Coordinate care transitions and arrange ancillary services (e.g., DME, infusion).
• Educate patients on discharge plans, medications, and follow-up appointments.
• Communicate with referral sources and healthcare providers to ensure timely care.
