Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a dedicated Care Transition Coordinator to facilitate smooth patient transitions from facilities to home health or hospice care. You will assess eligibility, coordinate care plans, and ensure ancillary services are arranged per agency standards, while educating patients and families.

Qualifications

• Minimum 1 year in home health or hospital case management.
• Active state RN/LPN/SW/PT licensure required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Achieve monthly production goals and MC admission targets.
• Coordinate care plans with facilities and clinicians.
• Evaluate patient eligibility for home health per guidelines.
• Educate patients and families about post-discharge plans.
• Document encounters in Home Care Home Base.
• Coordinate transfer orders and ancillary services.
