Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Care Transition Coordinator to manage patient transitions from facilities to home health or hospice, evaluate eligibility, coordinate care plans, and ensure services like DME and infusion are arranged per agency protocol.

You will liaise with referral sources and healthcare providers, support market growth, and maintain compliance while educating patients and families on care options and medication regimens.

Qualifications

• Minimum of one year of experience in home health or hospital-based case management.
• One to three years of experience in medical marketing or healthcare business development (preferred).
• Current and active licensure in the state of practice as RN, LPN, SW, or PT is required.

Responsibilities

• Achieve monthly personal production goals and Medicare-certified admission targets for assigned locations.
• Implement weekly, monthly, and quarterly strategies to increase market share within assigned facilities.
• Evaluate patients and physician orders for home care eligibility per Right of Choice guidelines.
• Conduct face-to-face patient transitions to educate patients and identify the primary care physician responsible for the plan of care.
• Coordinate transfer orders and ancillary services (e.g., DME, infusion) upon patient acceptance.
• Educate patients on post-discharge physician appointments and medication regimens.
• Act as liaison between the agency and healthcare providers for referred and transferred patients.
• Notify discharge planning of active patients and coordinate resumption of care as applicable.
