Job summary

Adoration Health in the United States seeks a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice. The role requires evaluating eligibility, coordinating care plans, and ensuring required services are arranged in line with agency protocols.

The CTC will liaise with referral sources, facilities, and healthcare providers, supporting market growth while ensuring compliant financial stewardship and high patient satisfaction through informed care

Qualifications

Minimum of one year in home health or hospital-based case management.
Preferred: 1–3 years in medical marketing or healthcare business development.
Current active licensure in the state of practice (RN, LPN, SW, or PT).
Valid driver’s license and reliable transportation required.

Responsibilities

Achieve monthly production targets and MC admissions for assigned locations.
Coordinate care plans and arrange services (DME, infusion) per agency protocols.
Educate patients and families about post-discharge orders and medications.
Liaise with referral sources, Executive Director, and Clinical Director to drive growth.
Document encounters in the agency system and ensure timely communication.
Maintain patient confidentiality in accordance with laws and policies.
