Job summary

Adoration Health in Fairfax, VA is seeking a Care Transition Coordinator (CTC) to facilitate patient transitions from facilities to home health or hospice care. Qualifications

• Minimum of one (1) year of experience in home health or hospital-based case management.
• One (1) to three (3) years of experience in medical marketing or healthcare business development.
• Current and active licensure in the state of practice as an RN, LPN, SW, or PT.
• Must possess a valid driver’s license, reliable transportation, and current auto insurance.
• Demonstrated understanding of home health eligibility criteria and Medicare/insurance coverage guidelines.

Responsibilities

• Achieve monthly personal production goals and MC admission targets for assigned locations.
• Manage sales and marketing expenses to ensure financial stewardship and ROI.
• Implement weekly, monthly, and quarterly market strategies to increase share in assigned facilities.
• Coordinate transfer orders and ancillary services (e.g., DME, infusion) for patients upon acceptance.
• Educate patients on post-discharge physician appointments, prescriptions, and medication regimens.
• Act as liaison between the agency and healthcare providers for newly referred patients.
