Adoration Health is seeking a Care Transition Coordinator (CTC) to facilitate patient transitions from healthcare facilities to home health or hospice care, evaluating eligibility and coordinating care plans including DME and infusion.

As a liaison among the agency, referral sources, and providers, you will manage documentation, educate patients, drive market growth, ensure financial stewardship, and uphold Medicare guidelines and patient satisfaction.

Qualifikationen

• Minimum of one year of experience in home health or hospital-based case management.
• Current active licensure as RN/LPN/SW/PT is required.
• Valid driver's license and reliable transportation.
• Strong knowledge of Medicare guidelines and coverage rules.

Aufgaben

• Achieve monthly production targets and Medicare-certified admission goals for assigned locations.
• Develop and implement strategies to grow market share within assigned facilities.
• Evaluate patient eligibility for home care per Right of Choice guidelines.
• Coordinate face-to-face transitions and educate patients on plans of care.
• Handle transfer orders and coordination of ancillary services such as DME and infusion.
• Educate patients on medications, prescriptions, and discharge procedures.
• Maintain documentation and communicate with discharge planning and case management teams.
