Job summary

BrightSpring Health Services, through its Adoration Health affiliate, is seeking a Care Transition Coordinator to streamline patient transitions from facilities to home health or hospice. The role coordinates eligibility, care plans, and ancillary services in line with agency protocols to ensure smooth, compliant care transitions.

The CTC will liaison with referral sources and providers, manage outreach, and support market growth while maintaining financial stewardship and patient satisfaction.

Qualifications

• Minimum of 1 year in home health or hospital-based case management.
• Experience in medical marketing or healthcare business development preferred.
• Active licensure in nursing, social work, or physical therapy as applicable.

Responsibilities

• Achieve monthly personal production goals and MC admission targets; manage sales expenses.
• Implement weekly/monthly/quarterly strategies to increase market share in facilities.
• Evaluate patients and orders for home care eligibility per guidelines.
• Conduct face-to-face transitions to educate and identify PCP responsible.
• Present needs to Executive Director and complete encounter documentation.
• Coordinate transfer orders and ancillary services (DME, infusion) after acceptance.
• Educate patients on post-discharge appointments and medication regimens.
• Serve as liaison between agency and healthcare providers for referrals.
• Notify discharge planning of active patients; coordinate resumption of care as needed.
• Provide follow-up feedback to case management on readmission decisions.
