Job summary

Adoration Health seeks a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice. You will coordinate care plans, assess eligibility, and liaise with referral sources and providers to ensure timely, compliant service delivery.

The role involves educating patients, coordinating DME and infusion services, and supporting market growth while maintaining financial stewardship and patient satisfaction.

Qualifications

• Minimum of 1 year experience in home health or hospital-based case management.
• Preferred 1–3 years in medical marketing or healthcare business development.
• Current active licensure in RN, LPN, SW, or PT in the state of practice.
• Respiratory Therapist certification and/or technical clinical program is a plus.

Responsibilities

• Meet monthly production goals and MC admission targets for assigned locations.
• Develop weekly, monthly and quarterly strategies to grow market share.
• Evaluate patient eligibility for home care per Right of Choice guidelines.
• Conduct face-to-face transitions and educate patients and families.
