Job summary

Adoration Health in the United States is seeking a Care Transition Coordinator (CTC) to facilitate patient transitions from facilities to home health or hospice care. You will evaluate eligibility, coordinate care plans, and arrange DME and infusion services in line with agency protocols and patient needs, while educating patients and families about the plan of care.

Qualifications

• Minimum 1 year in home health or hospital-based case management.
• Preferred 1–3 years in medical marketing or healthcare business development.
• Active licensure in RN, LPN, SW or PT in state of practice.
• RT certification is preferred.
• Valid driver’s license, reliable transportation, and auto insurance.
• Knowledge of home health eligibility criteria and Medicare/insurance coverage guidelines.

Responsibilities

• Achieve monthly personal production goals and MC admission targets.
• Lead strategies to increase market share within assigned facilities.
• Evaluate patients and physician orders for home care eligibility.
• Conduct face-to-face patient transitions and educate on plan of care.
• Coordinate transfers and ancillary services (DME, infusion).
• Educate patients on post-discharge appointments and medication regimens.
• Collaborate with Executive Director and Clinical Director to promote growth.
• Perform sales administration duties and maintain timely communication.
• Coordinate resumption of care after discharge.
• Maintain patient confidentiality.
