Qualifications

• Minimum 1 year of home health or hospital-based case management experience.
• Preferred 1–3 years in medical marketing or healthcare business development.
• Current valid licensure as RN, LPN, SW, or PT in the practice state.
• RT certification or clinical program is preferred.
• Driver’s license, reliable transportation, and auto insurance.
• Understanding of home health eligibility criteria and Medicare/insurance coverage guidelines.

Responsibilities

• Achieve monthly production goals and Medicare-certified admission targets for assigned locations; manage sales and marketing expenses.
• Implement weekly, monthly, and quarterly strategies to increase market share within assigned facilities.
• Evaluate patients and physician orders for home care eligibility according to Right of Choice guidelines.
• Conduct face-to-face patient transitions to provide agency education and identify the primary care physician responsible for the plan of care.
• Present identified patient needs to the Executive Director to obtain branch approval and acceptance; complete encounter documentation in Home Care Home Base.
• Coordinate transfer orders and ancillary services (e.g., DME, infusion) upon patient acceptance; educate patients on orders and services.
• Ensure all patient needs identified by the referral source are documented and addressed by the agency upon acceptance.
• Collaborate with the Executive Director and Clinical Director to promote growth aligned with referral sources and patient needs.
• Perform sales administration duties including BOA expense entry, PTO requests, and attendance at sales calls and in-services; maintain timely communication.
• Educate patients on post-discharge physician appointments, prescriptions, and medication regimens.
