Job summary

Adoration Health is seeking a Care Transition Coordinator (CTC) to facilitate patient transitions from facilities to home health or hospice care. The role coordinates care plans, evaluates eligibility, and aligns services with agency protocols, while liaising with referral sources and healthcare providers.

The CTC will support market growth, ensure financial stewardship, and enhance patient satisfaction through effective transitions and education.

Qualifications

• Minimum of 1 year of experience in home health or hospital-based case management.
• Must possess a valid driver’s license and reliable transportation.
• Demonstrated understanding of Medicare/insurance coverage guidelines.

Responsibilities

• Meet monthly personal production goals and MC admission targets for assigned locations.
• Implement sales/marketing plans to grow market share in facilities.
• Evaluate patient eligibility for home care per Right of Choice guidelines.
• Conduct face-to-face transitions and educate patients on plans of care.
• Coordinate transfer orders and ancillary services (DME, infusion) after acceptance.
• Educate patients on post-discharge physician appointments and medication regimens.
• Act as liaison between agency, referral sources, and healthcare providers.
• Complete encounter documentation and maintain timely communications via phone/email.
