Adoration Health, an affiliate of BrightSpring Health Services, seeks a Care Transition Coordinator to manage patient transitions from facilities to home health or hospice, coordinating care plans and ensuring services are arranged per agency protocols. The role includes evaluating eligibility, liaising with referral sources, and educating patients.

The CTC will drive market growth, handle sales-related administrative tasks, and ensure financial stewardship while maintaining patient satisfaction

Qualifikationen

• Minimum of one year of experience in home health or hospital-based case management.
• Current and active licensure in the state of practice as RN, LPN, Social Worker, or PT.
• Must possess a valid driver’s license, reliable transportation, and current auto insurance.

Aufgaben

• Achieve monthly production goals and MC admission targets for assigned locations.
• Implement strategies to increase market share within facilities.
• Educate patients and families about home care and care transitions.
• Coordinate transfer orders and ancillary services (DME, infusion) after patient acceptance.
• Maintain documentation and liaison with executives to promote growth.
• Manage sales administration duties including BOA expense entries and PTO requests.
