Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from healthcare facilities to home health or hospice care. You will evaluate eligibility, coordinate care plans, and arrange services including DME and infusion while liaising with referrals and healthcare providers.

The role includes achieving production targets, managing sales activities, and educating patients on post-discharge care.

Qualifications

• Minimum of one year of experience in home health or hospital-based case management.
• Active RN/LPN/SW/PT license required.
• Must possess a valid driver’s license and reliable transportation.
• Understanding Medicare/insurance coverage guidelines and eligibility criteria.

Responsabilités

• Achieve monthly personal production goals and MC admission targets.
• Implement strategies to increase market share within assigned facilities.
• Coordinate patient transitions and educate on care plans.
• Act as liaison between the agency, referral sources and healthcare providers.
• Complete encounter documentation and coordinate transfers for services.
