Job summary

NextStep Care is seeking a dedicated care manager to coordinate services for members across home and community-based supports, liaising with providers and families to ensure quality outcomes. You will assess needs, adjust care plans, and document interventions in a nonprofit, mission-driven setting.

Strong communication, empathy, and collaboration with care teams are essential as you support member engagement, continuity of care, and achievement of health goals.

Qualifications

• Bachelor's degree in social work or related human services field with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as a liaison between members and home and community-based service providers to facilitate quality and cost-effective care and outcomes.
• Provides education and support to members and their families regarding community resources and services.
• Develop effective, collaborative relationships with key stakeholders including primary care providers, managed care plan providers, home and community-based service providers (HCBS), formal/informal caregivers and families.
