Qualifications

• Bachelor’s degree in social work or related human services field required.
• 2+ years of work experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as a liaison between members and home/community-based service providers to facilitate care outcomes.
• Provides education and support to members and families regarding community resources and services.
• Develops effective relationships with primary care providers, managed care plans, HCBS providers, caregivers and families.
• Maintains regular communication with members via calls and home visits to monitor wellbeing and adjust care plans.
• Records and documents interventions and member information according to payer and organization guidelines.
• Collaborates with internal teams (Central Intake and Assessment) to ensure continuity of care.
• Identifies and resolves care plan variances and barriers to care.
• Monitors member engagement and behavior to support health and functional status.
• Participates in after-hours on-call rotation as assigned.
• Supports Quality Assurance and Education Team and Care Management Director to maintain service quality.
• Participates in Community Outreach per Quarterly Outreach plan.
• Supports training and onboarding of new team members.
• Promotes the System’s image through servant leadership and open communication.
• Participates in committees and project work as needed.
