Job summary

NextStep Care is seeking a dedicated Case Manager to support members with home and community-based services, ensuring quality and cost-effective care.

You will educate families on resources, collaborate with primary care providers, and document care plans accurately while maintaining strong communication with the care team. This full-time role includes after-hours on-call rotation and onboarding support for new staff.

Qualifications

• Bachelor’s degree in social work or related human services field with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as a liaison between members and providers to facilitate quality and cost-effective care and outcomes.
• Provides education and support to members and families regarding community resources and services.
• Develops collaborative relationships with key stakeholders including primary care providers and HCBS.
• Maintains regular communication with members via calls and home visits to monitor wellbeing and adjust care plans as needed.
• Records and documents interventions and member information accurately per guidelines.
• Collaborates with internal teams to ensure continuity of care for assigned members.
• Identifies and resolves care plan variances to ensure effectiveness of care plans.
• Participates in after-hours on-call rotation as assigned.
• Supports training and onboarding of new team members.
