NextStep Care is a nonprofit organization delivering community-based care with a mission to improve outcomes for members in Georgia. We seek a dedicated case manager to coordinate services, educate families, and advocate for access to necessary resources.

You will build relationships with primary care and home-based providers, monitor member wellbeing, and participate in after-hours on-call rotations, contributing to a team-focused culture that values servant leadership.

Formación

• Bachelor’s degree in social work or related field and 2 years of work experience are required.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.
• Ability to read and interpret documents, write routine reports and communicate effectively.

Responsabilidades

• Acts as a liaison between members and home and community-based service providers to facilitate quality and cost-effective care.
• Provides education and support to members and their families regarding community resources and services.
• Develops collaborative relationships with primary care providers, HCBS, and caregivers to ensure continuity of care.
• Maintains regular communication with members via calls and home visits to monitor wellbeing and adjust care plans.
