Job summary

NextStep Care is seeking a dedicated Care Manager to bridge members with home and community-based services. You will educate families, build strong provider relationships, and monitor care plans through regular outreach and documentation.

In this role, you’ll collaborate with care teams, address barriers to care, and participate in after-hours rotations to ensure continuous, high‑quality support for members’ health and independence.

Qualifications

• Bachelor’s degree in social work or related human services field required with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.
• Strong communication and interpersonal skills; ability to advocate for clients.

Responsibilities

• Acts as a liaison between members and home or community-based service providers to facilitate quality and cost-effective care.
• Provides education and support to members and their families regarding community resources and services.
• Develops effective relationships with primary care providers, HCBS, and care teams.
• Maintains regular contact with members via phone calls and home visits to monitor wellbeing and adjust care plans.
• Documents interventions and member information accurately per guidelines.
• Collaborates with internal teams to ensure continuity of care.
• Identifies and resolves care plan variances and barriers to care.
• Assesses member engagement and behavior to support health outcomes.
• Participates in after-hours on-call rotation as assigned.
• Supports training of new team members and promotes the organization’s image.
