NextStep Care in Georgia is seeking a dedicated Case Manager to coordinate home and community-based services for our members, aiming for quality outcomes and cost-effective care.

You will educate families, maintain clear records, collaborate with providers, and participate in after-hours rotations while supporting our mission-driven, nonprofit environment. This role requires a bachelor’s degree in social work and at least two years of experience.

Qualificações

• Bachelor’s degree in social work or related field.
• 2 years of work experience in social work, HCBS, healthcare or geriatrics preferred.
• Experience coordinating care and collaborating with providers.

Responsabilidades

• Acts as a liaison between members and home/community-based service providers to facilitate care outcomes.
• Provides education and support to members and families regarding resources and services.
• Maintains regular communication with members via calls and home visits to monitor wellbeing.
