Job summary

Monarch Communities in Southbury, CT, is seeking a compassionate Care Manager (CNA) to assist residents with daily living activities according to their care plans. This full-time role offers 32–37.5 hours per week across 6am–2pm and 2pm–10pm shifts in Assisted Living and Memory Care, with benefits including health insurance, retirement plans, and paid time off.

Qualifications

• Caregiving experience aligned with residents' care plans.
• Experience with elderly populations; dementia care preferred.
• Ability to assist with activities of daily living.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding as per care plan.
• Maintain safety, comfort, dignity, and confidentiality of residents.
• Escort residents to meals and activities and document services.
• Report changes in condition to supervisor.
• Assist with activities and daily living tasks as needed.
