Job Description

Led by the community's Director of Health and Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.

Schedules

• Part-time position (all shifts) in Assisted Living and/or Memory Care
• Salary Range: $19.00 - $22.00 hourly

Responsibilities and Duties

• Assist with self-administration of prescribed medication to residents and maintain related medical records, by way of queuing, prompting, and/or reminding; under the supervision of the RN and/or LPN.
• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Respect the needs of team members and residents by adhering to schedule and documenting attendance properly.
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Encourage and escort (when needed) residents to meals and activities
• Ensure all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts
• Document all services provided in EHR for your assigned residents during your shift
• Monitor and report any change of condition in a resident immediately to your supervisor
• Appropriately utilize and keep electronic documentation devices safe and secure within the community; efficiently document care/services provided during the shift.
• Participate in and attend all in-service trainings as scheduled
• Ensure compliance with all Federal, State, and local regulations, as well as community policies and procedures
• Provide basic housekeeping and/or laundry services when needed
• May be asked to obtain weights and vitals of residents and document same per RN request
• Other duties as assigned by supervisor, RN, and/or ED

Qualifications

• High School Diploma or GED. At least 1 year resident care experience required.
• Certified Nursing Assistant or Certified Home Health Aid
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• In this position, the associate will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
